What you'll do
- Perform resident assessments and monitor changes in condition
- Administer medications and treatments as ordered
- Participate in care planning and collaborate with physicians and other members of the healthcare team
- Communicate with physicians regarding changes in resident condition, diagnostic results, and other clinical concerns
- Supervise and provide direction to Certified Nursing Assistants (CNAs) and oversee resident activities of daily living and documentation
- Monitor residents and ensure care is provided according to their individual care plans
- Accurately document assessments, treatments, medications, and care in accordance with company policies and standards of practice
- Educate residents and their families regarding health conditions, treatments, medications, and other healthcare needs
- Complete required clinical documentation and forms in accordance with company policies and state and federal regulations
- Promote resident safety, dignity, privacy, and quality of life
What we're looking for
- Graduate of an accredited practical nursing program
- Current and valid LPN license in the state of employment
- Current CPR certification
- Long-term care or geriatric nursing experience is a plus
- Compassionate, and genuinely enjoys working with seniors
- Dependable and committed to providing excellent resident care
- A strong communicator and team player
- Comfortable working in a fast-paced long-term care environment
