What you'll do
- Perform comprehensive resident assessments and monitor for changes in condition
- Develop, implement, and evaluate individualized plans of care
- Administer medications, treatments, and other nursing interventions as ordered
- Provide skilled nursing care while promoting resident safety, dignity, comfort, and quality of life
- Participate in care planning and collaborate with physicians, nurse practitioners, therapists, and other members of the interdisciplinary team
- Communicate promptly with physicians and appropriate team members regarding changes in resident condition, diagnostic results, and other clinical concerns
- Supervise and provide direction to LPNs and Certified Nursing Assistants (CNAs) as appropriate
- Ensure resident care is provided according to established care plans, standards of practice, and facility policies
- Accurately document assessments, medications, treatments, progress notes, and other required clinical information
- Monitor and follow up on physician orders, laboratory results, and diagnostic testing
- Educate residents and their families regarding health conditions, medications, treatments, and ongoing care needs
- Respond appropriately to emergencies and changes in resident condition
- Support infection prevention, safety, quality assurance, and compliance initiatives
What we're looking for
- Graduate of an accredited nursing program
- Current and valid RN license in the state of employment
- Current CPR certification
- Long-term care or geriatric nursing experience is preferred
- Compassionate, and genuinely enjoys working with seniors
- Dependable and committed to providing excellent resident care
- A strong communicator and team player
- Confident in clinical decision-making and resident assessment
- Organized and able to prioritize multiple resident care needs
- Must maintain all required licenses, certifications, and continuing education
