RN MDS Coordinator
Own the MDS assessment process end to end, ensuring accurate, timely submissions that reflect the care our residents actually receive.

About this role
The MDS Coordinator makes sure the clinical picture on paper matches the care happening at the bedside. Accurate assessments drive appropriate reimbursement, regulatory standing, and — most importantly — care plans that genuinely fit each resident.
You will work closely with nursing, therapy, dietary, and social services to gather the full picture of every resident on your schedule.
What you'll do
- Complete and transmit MDS assessments within required timeframes
- Coordinate the interdisciplinary care plan process and conferences
- Audit supporting documentation for accuracy and completeness
- Track the assessment calendar and prevent late or missed submissions
- Partner with therapy and nursing on PDPM classification accuracy
- Support survey readiness and respond to documentation queries
What we're looking for
- Current, unencumbered South Carolina RN license (or compact privileges)
- Experience with the MDS 3.0 assessment process in a skilled nursing facility
- Familiarity with PDPM, care planning, and CMS submission requirements
- RAC-CT certification preferred, or willingness to obtain
- Strong attention to detail and comfort with clinical software
